Osteoporosis is a significant public health concern, particularly in aging populations, leading to fractures, decreased mobility, and reduced quality of life. While calcium and vitamin D have long been recognized as essential for bone health, emerging research suggests that potassium may play a crucial role in maintaining bone density and preventing osteoporosis. This manuscript explores the relationship between potassium and osteoporosis, delving into the mechanisms, epidemiological evidence, and potential therapeutic implications of potassium in bone health. Furthermore, the manuscript discusses the sources of dietary potassium, its impact on bone metabolism, and the future directions in research and clinical practice regarding potassium's role in osteoporosis management.

Skin diseases have recently become a major concern among people of all ages due to their highly visible symptoms and persistent and difficult treatment, which significantly impact their quality of life. Nigella sativa seeds, also known as "black seeds" or "kalonji," are one of the most commonly used herbal medicines due to their wide range of biological and pharmacological activities. It contains a wide range of bioactive constituents found in both fixed and essential oils. It has been used for hundreds of years as an alternative ethnomedicine to treat a wide range of skin conditions. N. sativa's dermatological applications in skin diseases are attributed to its potent antioxidant, anti-inflammatory, antimicrobial, and immunomodulatory properties, making it an intriguing skincare candidate. Several studies unravelled positive results associated with N. sativa on skin diseases. As N. sativa is the most studied medicinal plant, several preclinical and clinical studies have been conducted to establish its use in the treatment of various skin diseases. Thymoquinone has anti-inflammatory, antioxidant, and antibacterial properties, which mainly contributed to the treatment of skin diseases. In this context, the present review explores all the available studies on the association of N. sativa and its effect on treating skin diseases in light of recent studies and patents supporting its therapeutic applications.

The transport of drugs to the brain becomes a key concern when treating disorders of the central nervous system. Parkinsonism is one of the major concerns across the world populations, which causes difficulty in coordination and balance. However, the blood-brain barrier is a significant barrier to achieving optimal brain concentration through oral, transdermal, and intravenous routes of administration. The intranasal route with nanocarrier-based formulations has shown potential for managing Parkinsonism disorder (PD). Direct delivery to the brain through the intranasal route is possible via the olfactory and trigeminal pathways using drug-loaded nanotechnology-based drug delivery systems. The critical analysis of reported works demonstrates dose reduction, brain targeting, safety, effectiveness, and stability for drug-loaded nanocarriers. The important aspects of intranasal drug delivery, PD details, and nanocarrier-based intranasal formulations in PD management with a discussion of physicochemical characteristics, cell line studies, and animal studies are the major topics in this review. Patent reports and clinical investigations are summarized in the last sections.

In recent years, biologically synthesized metal nanoparticles have emerged as a dynamic field of research with significant implications for biomedical applications. This review explores the latest trends in the synthesis of metal nanoparticles using biological methods, encompassing plant extracts and microorganisms such as bacteria, yeasts, and fungi. These innovative approaches offer a sustainable, cost-effective, and environmentally friendly alternative to conventional chemical synthesis methods. Moreover, this review delves into the multifaceted biomedical applications of biologically synthesized metal nanoparticles. These applications include drug delivery systems, diagnostics, therapeutics, and imaging technologies, showcasing the versatility and promise of these nanomaterials in addressing contemporary biomedical challenges. In addition, the review addresses the critical issue of cytotoxicity, offering insights into the safety and viability of these biologically derived NPs for medical use. The exploration of recent trends and advancements in this field underscores the transformative potential of biologically synthesized metal nanoparticles in revolutionizing biomedical research and healthcare.

Vaccination is a potential public health solution for the prevention of infection. It reduces the severity of symptoms in case of COVID-19. Despite the availability of vaccines, some people are hesitant to be vaccinated. The objectives of the study were to measure the proportion of vaccine hesitancy among the peri-urban population and identify its determinants. An adult population of 303 from two peri-urban areas in the field practice area of Urban Health Training Centre, Rama Medical College were interviewed from 22nd February 2021 to 25th March 2021. Epicollect 5 was used for collecting data and STATA 16 was used for analysis. Multivariable logistic regression was applied to compute the adjusted odd ratio (95% confidence interval) to find out the determinants of vaccine hesitancy. Three Cs model guided tools of data collection and analyses. More than one fourth (28%) of the participants were vaccine-hesitant whereas 34.6% of participants had no confidence in the vaccine. Other reasons were complacency (40.6%) and convenience (35.9%). Vaccine hesitancy was significantly associated with gender [AOR = 2.40 (1.12-5.16)] and trust in government [AOR = 0.18 (0.08-0.45)] but no association with age group, political affiliation and source of information about the vaccine.  It is important to build the trust of people in vaccines, make it convenient and resolve the issues that are making them complacent. The health system needs to involve non-governmental organisations to reach out to those for whom there are issues of availability and approach.

Nanotechnology has emerged as an inspiring tool for the effective delivery of drugs to help treat Coronary heart disease (CHD) which represents the most prevalent reason for mortality and morbidity globally. The current study focuses on the assessment of the cardioprotective prospective ofanovel combination nanoformulation of sericin and carvedilol. Sericin is a silk protein obtained from Bombyx mori cocoon and carvedilol is a synthetic nonselective ß-blocker. In this present study, preparation of chitosan nanoparticles was performed via ionic gelation method and were evaluated for cardioprotective activity in doxorubicin (Dox)-induced cardiotoxicity. Serum biochemical markers of myocardial damage play a substantial role in the analysis of cardiovascular ailments and their increased levels have been observed to be significantly decreased in treatment groups. Treatment groups showed a decline in the positivity frequency of the Troponin T test as well. The NTG (Nanoparticle Treated Group), CSG (Carvedilol Standard Group), and SSG (Sericin Standard Group) were revealed to have reduced lipid peroxide levels (Plasma and heart tissue) highly significantly at a level of p < 0.01 in comparison with the TCG (Toxic Control Group). Levels of antioxidants in the plasma and the cardiac tissue were also established to be within range of the treated groups in comparison to TCG. Mitochondrial enzymes in cardiac tissue were found to be elevated in treated groups. Lysosomal hydrolases accomplish a significant role in counteracting the inflammatory pathogenesis followed by disease infliction, as perceived in the TCG group. These enzyme levels in the cardiac tissue were significantly improved after treatment with the nanoformulation. Total collagen content in the cardiac tissue of the NTG, SSG, and CSG groups was established to be highly statistically significant at p < 0.001 as well as statistically significant at p < 0.01, respectively. Hence, the outcomes of this study suggest that the developed nanoparticle formulation is effective against doxorubicin-induced cardiotoxicity.

OBJECTIVE: In the present study, an attempt has been made to develop SL-loaded transfersomal gel for the effective treatment of delayed wound healing. SIGNIFICANCE: The wound healing process consists of a complex series of biochemical events and changes in cellular activity that restore the integrity of the skin and the subcutaneous tissue. Sesamol (SL), which is a natural phenolic compound, is known for its antioxidant properties, anti-inflammatory properties, and wound-healing abilities. METHODS: A thin-film hydration method was used to prepare SL-loaded transfersomes. Different formulations containing Tween-80 and Span-80 as edge activators were prepared and optimized. Various characteristics of vesicles were assessed, such as size, shape, loading efficiency, deformability, and in vitro skin penetration. The optimized formulation was then incorporated into 1% carbopol 940 gel. An in vivo wound healing potential of the selected formulation was assessed by an excision wound model. RESULTS: The SL-loaded transfersomal gel displayed improved skin penetration and better skin deposition. Wound healing studies showed that the highest wound contraction was observed with SL-loaded transfersomes. Following 21 days of application of the transfersomal gel, a marked improvement in skin histological architecture was found. CONCLUSION: The study findings suggest that transfersomal gel has great potential as a therapeutic option in wound healing.

Diabetic foot wounds (DFUs) are counted as one of the most common microvascular complications associated with poorly controlled and chronic diabetes mellitus. It confers a serious challenge to clinical practice, surmounting hyperglycemia-induced disturbance in angiogenesis and endothelial dysfunction, with limited fruitful intervention to control the manifestations of DFUs. Resveratrol (RV) can improve endothelial function and has strong pro-angiogenic properties for the treatment of diabetic foot wounds. The present study aims to design an RV-loaded liposome-in-hydrogel system to effectively heal diabetic foot ulcers. A thin-film hydration method was used to prepare RV-loaded liposomes. Liposomal vesicles were assessed, for various characteristics such as particle size, zeta potential, and entrapment efficiency. The best-prepared liposomal vesicle was then incorporated into 1% carbopol 940 gel to develop a hydrogel system. The RV-loaded liposomal gel showed improved skin penetration. To assess the efficacy of the developed formulation, a diabetic foot ulcer animal model was used. The topical application of the developed formulation significantly reduced blood glucose and increased glycosaminoglycans (GAGs) to improve ulcer healing as well as wound closure on day 9. Faster re-epithelization, proliferation of fibroblast, formation of collagen, and reduced inflammatory cell infiltration at the wound site were also noted. Results indicate that RV-loaded liposomes in hydrogel-based wound dressing significantly accelerate wound healing in diabetic foot ulcers by restoring the altered wound healing process in diabetics.

BACKGROUND: An RP-HPLC (Reverse Phase-High-performance liquid chromatography) method for the quantitative estimation and validation of the plumbagin in the methanolic fraction of Plumbago zeylanica L. was developed. METHOD: For achieving good separation, the RP-HPLC method was carried out with reverse phase C18 column, using methanol and water as mobile phase in the ratio of 65:35 (v/v), at the flow rate of 1 mL/min. The detection wavelength was set at 265 nm. RESULTS: The retention time of plumbagin was found at 7.5±0.2 min. The coefficient of determination of plumbagin was found to be (r2) 0.9985 and equation Y=23148x+4327. The LOD and LOQ were found to be 34.06 and 113 ng/mL, respectively. CONCLUSION: The developed method was accurate, specific, precise, and reproducible. This RP-HPLC may be useful for quantitative estimation of the chemical constituents present in the plant extract as well as the quality assessment of the herbal product.

BACKGROUND: Fungal infections are one of the most common dermatological issues worldwide. Candida species-caused fungal infections are frequent on the cutaneous surface. Eberconazole (EBZ) has the strongest antifungal action against Candida spp., the major source of fungal infections. METHOD: In the present study, the cold method followed by probe sonication was used to create EBZ-loaded ethosomal dispersion. The solubility of ethosomes in different lipids and surfactants was used to choose these components. Under magnetic stirring, the dispersion was absorbed into a carbopol 934 gel. In vitro antifungal activity was performed using the Agar well diffusion method, and their topical effectiveness against pathogenic Candida albicans was compared to that of a marketed formulation containing EBZ. RESULTS: Eberconazol incorporated into gel displayed sustained release in an in vitro release assay. Based on the zone of inhibition diameters, EBZ formulation was determined to be efficient against C. albicans when compared to the commercialized cream and plain gel. CONCLUSION: Based on these findings, the current study found that EBZ possesses significant antifungal efficacy against C. albicans.

Background: COVID-19 vaccination is still a matter of concern among the public since its inception. Primary care physicians being in prime position can share accurate and ample information about COVID-19 vaccination so we estimated proportion and determinants of adverse events following immunization (AEFI) with Covishield vaccine, vaccination and reasons of non-vaccination. Methods: A cross-sectional study was conducted from January to April 2021. Data collection was done by using data capture tool Epicollect-5. Regression analysis was performed to evaluate the association of AEFI and vaccine acceptance with various demographic variables. Results: Of 520 subjects, 408 got vaccinated, and of these 125 (30.6%) developed AEFI. Females without AEFI had lower median age than females having AEFI. Only religion (aOR = 5.311; 95% CI: 1. 216-23.1) was significantly associated with AEFI. Education (aOR = 0.399; 95% CI: 0. 199-0.799), marital status (aOR = 0.459; 95% CI: 0. 245-0.858), and religion (aOR = 3.874; 95% CI: 1. 96-7.648) were significantly associated with vaccination. Most common local AEFIs were inflammation (46; 36.8%) followed by lump (10; 8%) at the site of injection. Most common systemic AEFI were fever (87; 69.6%), feeling unwell (65; 52%), generalized weakness/fatigue (30; 20%), tiredness (26; 20.8%), flu-like symptoms (12; 9.6%), dizziness (10; 8%), headache (8; 6.4%) and gastrointestinal events (7; 5.6%). Conclusion: Most of the AEFI were mild and transient, resolved without any medical management. This study warrants active reporting of AEFI, public release of safety, and efficacy data. Primary care physicians can play a pivotal role by targeted awareness campaigns and trust-building activities to alleviate fear and anxiety related to vaccine.

Psoriasis is an autoimmune disease characterized by erythematous, scaly patches on the skin. It can be effectively managed with topical therapies since they deliver drugs to target sites of disease efficiently and can minimize systemic side-effects while ensuring high patient compliance. However, conventional topical formulations are ineffective in treating psoriasis due to their poor percutaneous penetration and inability to reach deeper layers of the skin. Thus, it is important to explore new approaches for managing psoriasis safely and effectively while also maintaining patient compliance without compromising safety. Over the last few decades, a variety of nanocarriers have been extensively investigated as a new approach to delivering drugs to the skin that are effective against psoriasis. These nanocarriers are notable for their therapeutic effectiveness, increased localization of medication in the skin, and reduced side-effects. The purpose of this review is to explore the recent advances in polymer-based, lipid-based, metallic, and microneedle-based novel nanoformulations of antipsoriatic drugs. There have been detailed discussions about several nanocarrier systems including nanoemulsions, liposomes, nanostructured lipid carriers, ethosomes, solid lipid nanoparticles, micelles, gold nanoparticles, silver nanoparticles, and microneedles. In a nutshell, nanoformulations are considered a promising avenue for psoriasis treatment since they offer better penetration, targeted delivery, and enhanced safety and efficacy.

Diabetic foot ulcers (DFUs) are the most common complications associated with diabetes mellitus. DFUs are displayed as open sores or wounds located on the bottom of the foot as a secondary complication of diabetes mellitus (DM). DFUs are associated with significant morbidity and mortality and can subsequently lead to hospitalization and lower limb amputation if not recognized and treated on time. An immense challenge to conventional treatments is caused by the chronic nature of diabetic foot syndrome and it has led to the emergence of nanotechnology-based therapeutics. The greatest advantages of these nanotherapeutics are their unique biological, chemical, and physical properties. The present review highlights the augmentation of bacterial infections relating to delayed healing of DFUs and the potential of nanotherapeutics such as polymeric nanoparticles, metallic nanoparticles, siRNA-based nanoparticles, lipid nanoparticles, and nanofibers in accelerating wound healing in diabetic foot ulcers.

BACKGROUND: In the recent years, Micelles represent a promising carrier for the treatment and diagnosis of cancer. Architecturally, micelles are self-assembled nanosized colloidal aggregates prepared from amphiphilic surfactant with a hydrophobic core and hydrophilic shell. Such a composition makes them a potential carrier for delivery of hydrophobic anticancer drugs with in their core. METHODS: Micelles have received increasing interest as an enhanced permeability and retention (EPR) targeted drug delivery systems for cancer treatment. Micelles can be modified to contribute various attractive properties, for instance, active targeting, stimuli-responsiveness. They have also proven their ability in drug targeting to tumor tissue, enhanced drug accumulation, drug stabilization, tissue penetration, prolong circulation, in vivo biocompatibility, biodegradability and reduced side effects. Micelles have displayed a vital role in multidrug delivery for cancer therapy. RESULTS AND DISCUSSION: The aim of the present review is to provide an overview on the status of micellar nanoformulations for anticancer agents, including their pre-clinical and clinical researches. Emphasis is placed on presenting the newer strategies to enhance the therapeutic efficacy of anticancer drug at the target site. The type of co-polymers used and methods for the preparation of micelles are also highlighted in the paper.

The variation in alkaloid metabolites (colchicine and gloriosine) was found significant in the nine germplasms of G. superba (L.), collected from Central India. The maximum content of colchicine and gloriosine was in NBG-15 (Chitrakoot, M.P) and NBG-13 (Bheraghat, M.P). The phenolic acids viz. quercetin and kaempferol was first ever quantified in G. superba tuber. Cluster analysis on chemical variability (colchicine and gloriosine content) results in the identification of three elite germplasm(s). The radical scavenging potential was also found promising in the selected elite germplasm viz. NBG-13, NBG-14 and NBG-15. Further, the protein denaturation potential of elite chemotypes was found at par with standard colchicine. The study will aid in site specific exploration of high metabolite yielding chemotype(s) with validated pharmacological action to meet out the industrial demands. This will also promotes the commercial cultivation of species for socio economical upliftment in the area having similar phyto geographical conditions.

BACKGROUND: In dermatology, the topical application of liposomes has proven to be of therapeutic value. Many drugs encapsulated into liposomes have shown enhanced skin penetration. METHODS: Liposomes are lipid based spherical vesicles. In structure and composition, they resemble cell membrane. Because of their amphipathic nature, they can be used as carriers for hydrophilic as well as lipophilic therapeutic agents. These liposomal systems can easily get integrated with the skin lipids and maintain the desired hydration conditions to improve drug penetration and localization in the skin layers. RESULTS: Considering the need for topical delivery and the promising potential of liposomes, an attempt has been made to explore the recent advances of liposome-based formulations in dermatological applications. CONCLUSION: This patent summarizes the recent findings of liposome-based formulations for dermal delivery of drugs.

Fungal infections have become a subject of great concern and the incidence of fungal infections is increasing, presenting an enormous challenge to healthcare professionals. Since most of the fungal infections are occurring over the skin, the treatment option of these infections always involves topical application. However, in topical delivery drug reaches into systemic circulation through different barriers of skin. Nevertheless, due to the low permeability, skin restricts the movement of many drugs. Hence, a delivery system is required, which deliver the medicament into the skin layers or through the skin and into the systemic circulation. Ethosomes or Soft malleable vesicles are the novel lipid vesicular carrier that offer improved skin permeability and efficient delivery due to their structure and composition. They contain high concentration of ethanol, which increases the fluidity of the skin. Therefore, in the present paper, we have explored the utility of ethosomal systems in the topical treatment of fungal infections. Structure, compositions types, mechanism and techniques of preparation of ethosome also discussed in the paper.

Diabetic retinopathy is a microvascular complication in diabetes that affects eyes and is responsible for most visual impairment in diabetic patients. Diabetic retinopathy affects up to 80% of those who have had diabetes for 20 years or more. At least 90% of new cases could be reduced with proper treatment and monitoring of the eyes. The longer a person has diabetes, the higher his or her chances of developing diabetic retinopathy. Hence, it compels need for its prevention and cure. There is an increasing interest in natural products in pharmacotherapy as the chemical diversity of natural products has better matches than the diversity of synthetic compounds. The current review summarises the potential of leading traditional herbs like Azadirachta indica, Ginkgo biloba, Anisodus tanguticus, Pinus pinaster, Salvia miltiorrhiza, Stephania tetrandra and Gymnema sylvestre in the management and potential reversal of DR-related pathogenesis. It also discusses the probable mechanism of actions, which are based on epidemiological, in-vitro and in-vivo studies carried out within past few years. Graphical Abstract.
